In order to prevent the breakdown of fat cells after breast surgery and thus deformations, the removed fat must contain as many vital fat cells as possible. To obtain this, we carry out a modified version of the Coleman technique. The fatty tissue is obtained in a sterile manner and then centrifuged after several filter processes before we insert it into the breast for enlargement.

The Breast Augmentation Procedure with the Patient's own fat
The procedure itself takes about two to four hours. If you wish to increase the volume considerably, several sessions will be necessary. The operation can be performed under local anesthesia, but if desired, also under general anesthesia. First of all, we remove vital fatty tissue at suitable places and prepare it accordingly. The tissue is then injected into the breast.
Although the technique of autologous fat transplantation for breast augmentation appears simple and not very complicated as such, some important details must be considered when carrying out this procedure, otherwise permanent and stable result cannot be guaranteed. Patients are now being recruited by a plethora of cheap offers, most of which do not take into account the most important principles of autologous fat transplantation. One of these important principles is the fact that fat cells are vital tissue at the time of transplantation. This in turn derives some consequences for dealing with the so-called aspirate. The aspirate, represents the tissue that is removed under the liposuction.
The aim is to gain as large a volume of vital fat cells as possible in order to transplant them back into the body. There are also different methods that describe how to use the fat cells obtained for breast augmentation. In our private clinic for plastic and aesthetic surgery in Berlin, we perform a modified procedure using the Coleman technique. The fatty tissue is obtained in a sterile manner and then centrifuged after several filtration processes, in order to collect it in individual syringes for reinsertion into the body.
Breast augmentation with the patient’s own fat is based on the transplantation of fat, also known as lipofilling. When used correctly, autologous fat transplantation proves to be not only a permanent but also a natural method. The prerequisite for this is that vital tissue is removed and prepared accordingly before it is injected into the body. Only then is the method low-risk and leads to the desired results.
A gentle procedure and careful processing is urgently required when handling the removed fat cells by liposuction. The most common mistake made when using the patient’s own fat for breast augmentation is incorrect handling of fat cells as the basic substance for breast augmentation. There is a constant debate about whether autologous fat should be enriched with a particularly high number of stem cells before it is transplanted into the breast. Plastic surgeons who hold this opinion, however, disregard the basic architecture of the breast with its gland and surrounding fatty tissue. It is true that the female bust consists mainly of glandular tissue and the surrounding fatty tissue. However, the proportion of stem cells in fatty tissue is very low here.
Therefore, it does not make sense to focus on a substrate consisting mainly of stem cells when transplanting fatty tissue for breast augmentation. Current scientific studies show that the largest proportion of fatty tissue remains alive at the transplantation site and survives if a natural composition of fatty tissue, which consists only to a small extent of stem cells, was transplanted during breast augmentation with autologous fat. In addition, there are critical studies dealing with the induction of cancer in the female breast in connection with the transfer of stem cells for breast augmentation using the autologous fat method. To summarise current scientific data, the transplantation of suctioned fat tissue in a natural composition is the only safe and reliable procedure for breast augmentation with autologous fat.
Depending on the desired breast size and the initial situation of the existing mammary gland, a single session of autologous fat transplantation for breast augmentation may or may not be sufficient. Since not all of the transferred fatty tissue is accepted by the body and incorporated into the breast during breast augmentation with autologous fat, we speak of resorption rates in terms of success with the autologous fat method. In this case a successful transfer can be regarded in terms of the absorbed fatty tissue for breast augmentation with a healing rate of 66-80 %. The transplantation rates of successfully transferred own fat for breast augmentation are below 50% only in very rare cases, such as heavy smokers or those with poor initial immune system conditions. Therefore, depending on the objective, several sessions for liposuction and transplantation of the patient’s own fat must be planned.
It is not uncommon for patients to come to use with an initial situation of an A-cup and outline their goals of a full C-cup. Two sessions are usually necessary here, as the fatty tissue introduced during one session is not sufficient for breast augmentation. As a rule, about 200g of fatty tissue per breast can be transferred effortlessly in one session. The percentage and thus the loss rates of the fat tissue successfully incorporated into the breast during breast augmentation with autologous fat depends to a large extent on how the fat tissue obtained from liposuction is handled. Therefore, a certain amount of time is always necessary due to the necessary care, precision and quality assurance in the transfer of autologous fat after liposuction for breast augmentation. In order to give our patients an overview of the desired bust size in relation to their own body before an operation, we also create before-and-after pictures with their own fat as a simulation before the breast enlargement. Using the 3D visualisation software Crisalix, the enlarged breast is simulated individually for each patient. Photos of the patient serve as a basis, which are translated into 3D by the software. The individually created before and after pictures give our patients a very precise idea of what the result of their breast enlargement with their own fat will look like.
The results of breast augmentation with autologous fat usually last a lifetime. However, with this method, the final result is only visible after about six to twelve months. During this time, the initial volume is reduced by about thirty to forty percent. The final result is then permanently visible. For this reason, we always carry out a higher volume transfer of fat than initially desired by the patient, in order to take into account the loss rate that occurs due to resorption processes.
If the first volume reduction is achieved by resorption of a certain percentage of the inserted fat tissue in this method of breast augmentation, the percentage of the inserted fat tissue remains stable in the long term. However, even with the transplanted fatty tissue, age related changes occur over the years. These include hormonal changes but also changes in the overall tissue consistency in terms of tension and shape of the breast. Therefore, a decrease in the amount of the introduced autologous fat is not only a logical consequence, but also part of the natural appearance of autologous fat transplantation for breast augmentation compared to the procedures in which silicone implants are inserted. In the case of drastic weight reduction, the inserted fatty tissue is also reduced at the breast gland. However, the fat cells are not completely lost, so that when weight is gained again, the fat cells increase in volume, once again achieving the previous volume effect. Meaning the method of autologous fat transplantation for breast augmentation is a permanently stable procedure.
Since not every body fat is equally suitable for breast augmentation with autologous fat, it is important to have the procedure performed only by a highly skilled and experienced plastic surgeon. This person must select the ‘right’ fat, extract it in a sterile manner, prepare it and then inject it into the breast at the appropriate point, which requires experience. The sterile extraction of suitable tissue and the subsequent injection into the breast is the high standard to which every plastic surgeon should adhere. If breast enlargement with autologous fat is performed by an experienced specialist, the side effects and risks are usually relatively low. One complication of breast enlargement with autologous fat is oil cysts, which occur when too much fat is injected into the breast. Breast augmentation with autologous fat not performed by a specialist involves considerable risks. The fat cells cannot grow and form a cyst. In order to minimize the risk of oil cysts, we only perform breast augmentation with the patient’s own fat with a large volume over several sessions.
Costs
The cost of breast augmentation with autologous fat is very close to the cost of breast augmentation with silicone implants. This is due to the fact that the procedure is very complex and time-consuming in terms of the extraction and preparation of the fatty tissue, as opposed to an already finished implant, which is readily available for breast augmentation. The operative and time expenditure is therefore higher. In contrast to breast augmentation with implants, breast augmentation with the patient’s own fat often requires follow-up procedures that incur additional costs. The cost of breast augmentation with a silicone implant is often lower than the cost of a transplant of autologous fat. This does not make sense at first, as the material costs for the silicone implant are saved. In addition, the transplantation of autologous fat for breast augmentation involves material costs resulting from the processing of the aspirate obtained from liposuction. Therefore, it is not enough to simply collect the fat from the liposuction and inject it back into the patient or breast in its pure form. Such a procedure can have fatal consequences for the mammary gland, as scarring, oil cysts and low incorporation rates or high resorption rates of the inserted fatty tissue can result. In order to make the costs of breast augmentation with autologous fat reasonable for the patient, we offer complete packages depending on the planning of the target size and the resulting series of procedures. The costs of the initial consultation always depend on the type and extent of the treatment. In principle, the scale of fees for doctors applies to the consultation hours.
