Lip filler techniques: methods, differences and which one suits you

When it comes to lip augmentation, there is no single correct method. Instead, there are different techniques that are used depending on the starting situation, the desired lip shape and the individual anatomy. Some people want more volume, others a clearly defined contour or a subtle lifting effect. What matters is therefore not only the material used, but above all the injection technique. Different shapes are created by different placements and dosages of the filler. This article explains the most common methods and their differences, and shows which lip filler technique can make sense in an individual case.

The most important facts at a glance

Lip augmentation mainly uses hyaluronic acid, autologous fat or, in certain cases, Botox. The most frequently used techniques for lip injections are based on different injection patterns such as vertical, linear or fan-shaped placements. Target groups differ depending on whether volume, contour or a natural effect is desired. Depending on the material, the durability varies between several months and permanent (autologous fat). Risks can be reduced considerably by qualified practitioners and high-quality materials.

Which materials are used for lip augmentation?

The choice of material has a decisive influence on the result, the durability and the effort involved in the treatment. Not every method is suitable for every anatomical starting point or every aesthetic wish. Different materials allow for different effects – from temporary volume build-up and visual accentuation through to longer-lasting solutions. In aesthetic practice, three variants are used above all: hyaluronic acid as the classic lip filler for volume and contour, the so-called lip flip with Botox for visual accentuation without any gain in volume, and autologous fat as a potentially more permanent alternative. Each material has its own advantages and disadvantages, different durabilities and specific fields of application. Which method makes sense therefore always depends on the individual goals and on the respective lip anatomy.

Hyaluronic acid is the standard material for lip corrections. It is used most frequently because it is mouldable, well tolerated and reversible. The material is suitable both for building up volume and for defining contours. One key advantage is its controllable durability. The hyaluronic acid is broken down by the body over time. This means that shape and volume can be adjusted if required. Different degrees of cross-linking make it possible to tailor the consistency individually to the lip region.

With a lip flip, no volume is injected. Instead, a small amount of botulinum toxin is placed into the upper lip muscle. This relaxes the muscle slightly so that the lip rolls minimally outwards. The result is a visual enlargement without any actual build-up of volume. This method is particularly suitable for people who want a subtle change or who have a very narrow upper lip.

Lip augmentation with autologous fat is a more complex alternative. The body's own fat is harvested, processed and injected into the lips. The advantage lies in the potentially more permanent effect. However, the procedure is more complex and involves greater effort. In addition, it cannot be guaranteed how much of the transplanted fat will remain in the long term.

Lip augmentation: techniques with hyaluronic acid at a glance

Hyaluronic acid is the most frequently used material for lip augmentation. However, what is decisive for the result is not only the product itself, but above all the chosen injection technique. Different techniques lead to different shapes, projections and contours. While some methods specifically build up volume, others concentrate on definition, symmetry or a lifting effect. Which technique makes sense in an individual case depends on several factors: lip anatomy, existing volume, skin quality and the desired aesthetic result. In the following, the most important lip filler techniques are explained in detail.

The Russian Lips technique is one of the best-known modern types of lip augmentation. Its characteristic feature is the vertical injection technique, in which the hyaluronic acid is introduced from the lip border upwards towards the centre of the lip. Unlike classic methods, the volume is not primarily projected forwards but rather built up upwards.

The aim of this technique is a heart-shaped, clearly defined lip contour with an accentuated lip centre. The upper lip is given a pronounced shape, while the lateral areas remain more restrained. This creates a structured look without strong protrusion. The technique requires a great deal of precision, as the vertical placement is anatomically demanding. It is particularly suitable for people who want defined lip shapes without aiming for an excessively voluminous result.

The tenting technique – also known as lip tenting – is likewise based on vertical injections, but with a stronger focus on even elevation. Small amounts of hyaluronic acid are placed selectively in order to lift the lip in a targeted way.

The result is often a gentle lifting effect that makes the lips look fresher and more structured. This method is particularly suitable for lips that appear flat or for people whose lips show little projection in profile. Compared with Russian Lips, the result is usually more natural and less trend-driven. The tenting technique is one of the techniques that aim for a harmonious improvement without forcing a striking shape.

The keyhole technique is a special variant in which the centre of the lip is accentuated more strongly, while a small gap remains along the central line. This creates the impression of a „keyhole“ in the middle of the lips. This technique is one of the trend-oriented types and is often chosen when a particularly sensual appearance is desired. The volume concentrates on defined areas, while other zones are deliberately accentuated less. Not every lip anatomy is suitable for this method. Careful planning is decisive in order to achieve a harmonious overall appearance and to avoid overcorrection.

The linear injection technique is a classic and proven method. The hyaluronic acid is injected along defined lines into the vermilion or the lip border. The aim is an even build-up of volume as well as a clearer contour. This technique is particularly suitable for people who want a moderate enlargement or an improvement of the lip shape. It offers good control over the volume and allows for an even result. As the placement is very targeted, the linear technique can be adapted flexibly to different shapes. It is one of the most frequently used methods in aesthetic practice.

With the fan technique, the hyaluronic acid is distributed from a single entry point in several fan-shaped movements. This method allows the material to be distributed evenly over a larger area. It is particularly suitable for building up volume in the lateral lip area or close to the corners of the mouth. Thanks to the fan-shaped placement, the result can look softer and more natural. The fan technique can be carried out with a needle as well as with a blunt cannula. The latter can reduce the risk of haematomas and is frequently used for larger areas.

The vertical technique is based on several vertical entry points at which small amounts of hyaluronic acid are distributed evenly. The aim is a harmonious, balanced lip volume without strong projection. This technique is particularly suitable for people who prefer a subtle and natural result. Instead of a pronounced increase in volume, the focus here is on the structural improvement of the lips. The even multi-point injection can reduce the risk of small lumps forming, provided the technique is carried out precisely.

Lip augmentation with autologous fat

Lip augmentation with autologous fat is an alternative to treatment with hyaluronic acid. Unlike with fillers, the body's own fatty tissue is used here, which is previously harvested from another part of the body – for example from the abdomen or the thighs. After processing, the harvested fat is injected into the lips. A key advantage of this method is its potentially longer durability. As it is the body's own tissue, part of the transplanted fat can heal in permanently. However, it is not possible to predict exactly beforehand which proportion will remain in the long term. Part of the fat is broken down again by the body in the first few weeks.

Compared with classic lip augmentation using hyaluronic acid, the effort involved is considerably higher. It is a more invasive procedure that includes fat harvesting, sterile processing and subsequent injection. This extends the duration of the treatment, and the recovery period can also be more intense. Lip augmentation with autologous fat is particularly suitable for people who want a longer-term solution and are prepared to accept the additional surgical effort. Careful consultation is especially important here in order to set realistic expectations.

Lip flip with Botox

The so-called lip flip is a minimally invasive method that differs clearly from classic lip augmentation. Instead of introducing volume into the lip, a small amount of botulinum toxin is injected into the upper lip muscle. The targeted relaxation of this muscle causes the upper lip to roll slightly outwards. This visually creates the impression of a fuller lip, even though no additional volume is built up. This method is particularly suitable for people with a very narrow upper lip or for those who want a subtle change.

The durability of a lip flip is usually a few months, as the effect of Botox is limited in time. Compared with hyaluronic acid, the effect is more subtle and less shaping. The method is therefore not suitable for pronounced volume wishes. However, the lip flip can be a sensible option if only a slight accentuation is desired or if there is uncertainty about fillers.

Cannula or needle: which technique makes sense?

With these techniques, it is not only the placement of the filler that plays a role, but also the instrument used. In principle, there are two options: the classic sharp needle or the blunt cannula. The needle allows for a very precise placement of the material. It is particularly suitable for fine contour work or selective corrections. However, with every injection using a needle there is a higher risk of small bruises.

The blunt cannula, on the other hand, has a rounded tip and tends to push the tissue aside rather than piercing it. This can reduce the risk of haematomas. Cannulas are frequently used for larger areas or when building up volume in the lip region. Which method makes sense depends on the chosen technique, the treatment goal and the individual anatomy. In many cases, a combination of both instruments is used in order to ensure both precision and safety.

Which technique suits me?

The question of the right technique for lip augmentation cannot be answered across the board. What is decisive is the individual lip shape, the existing volume, the skin structure as well as personal aesthetic ideas. Anyone who wants a clear contour with an accentuated centre could benefit from the Russian Lips technique. For a natural, even volume, a linear or vertical technique can make sense. Trend-based shapes such as Keyhole Lips, on the other hand, are only suitable for certain anatomical conditions.

The question of naturalness versus strong volume also plays a role. Realistic expectation management is essential in order to avoid later dissatisfaction or unnecessary corrections. The experience of the practitioner is a decisive factor here, as only a well-founded analysis of the lip anatomy can determine the appropriate technique.

Preparing for lip augmentation

Good preparation contributes substantially to a safe course of treatment with few complications. A detailed medical history should be taken before the treatment. Existing illnesses, allergies or the intake of medication – in particular blood thinners – must be reported to the treating doctor. Intense sun exposure, tanning beds or chemical peels should be avoided for at least 24 hours before the treatment. Make-up in the mouth area should also be avoided on the day of treatment in order to minimise the risk of infection. A professional consultation clarifies not only technical questions but also realistic expectations. Possible risks, side effects and the appropriate technique are discussed individually.

Procedure of the lip treatment

The procedure of a lip treatment begins with the individual consultation. The desired lip shape, the material and the appropriate injection technique are determined. The treatment area is then disinfected. Depending on preference and sensitivity, a local anaesthetic can be applied in the form of a cream or an injection. Afterwards, the hyaluronic acid or the corresponding material is injected precisely using a needle or a cannula.

After the treatment, the lips are checked and, if necessary, gently modelled. A short period of observation ensures that no immediate complications occur. Finally, patients receive instructions on aftercare in order to reduce swelling and stabilise the result.

Aftercare: do's & don'ts after lip augmentation

Correct aftercare is a decisive part of every lip treatment. Regardless of which techniques were used, behaviour in the first few days significantly influences the final result as well as the risk of side effects. Immediately after the injection, the tissue reacts sensitively. Swelling, redness or a feeling of tension are normal reactions that usually subside within a few days. Structured aftercare supports the even distribution of the filler, reduces swelling and helps to detect possible complications at an early stage. The lips should be treated gently especially during the first 48 hours, as the material has not yet fully stabilised. Mechanical pressure, a strong increase in blood circulation or heat can affect the result.

Gentle cooling in the first hours after a hyaluronic acid treatment can help to reduce swelling. However, ice should not come into direct contact with the skin; a soft cooling pad or a cloth should be used instead. Drinking enough supports general regeneration. As hyaluronic acid binds water, the tissue benefits from a good fluid intake. From the third day onwards – provided it is medically recommended – a light, careful massage can make sense in order to stimulate blood circulation and even out small irregularities. This applies exclusively to hyaluronic acid treatments and only under medical guidance. In addition, follow-up appointments should be kept. A professional check-up makes it possible to detect any irregularities at an early stage and to correct them if necessary.

In the first few days after lip augmentation, avoid anything that strongly stimulates blood circulation or puts pressure on the freshly treated tissue. This includes alcohol, sport, sauna and tanning beds as well as intense sun exposure, because heat and increased circulation can make swelling worse. Make-up in the lip area should be left off for the first 24 hours in order to keep the risk of infection as low as possible. Planned dental treatments and permanent make-up of the lips should also be postponed until the hyaluronic acid sits stably in the tissue – both mean pressure and stretching in the treated area. The same applies to massaging or pulling at the lips while the material is still settling.

In short, the following should be avoided in the first few days:

  • Alcohol
  • Sport
  • Sauna
  • Sun
  • Make-up in the lip area
  • Dental treatment
  • Permanent make-up

Side effects and risks of lip augmentation

As with every medical procedure, side effects can also occur with lip augmentation. Most reactions are, however, mild and temporary. Typical side effects include swelling, redness, small haematomas or a feeling of tension. These symptoms usually subside within a few days. In rare cases, more pronounced swelling or palpable hardening can occur. In such cases a medical check-up makes sense in order to rule out possible causes such as an uneven distribution of the material. When carried out professionally and with high-quality materials, serious complications are rare. Serious but rare risks include circulatory disorders, allergic reactions or inflammatory processes. It is particularly important to take unusually severe pain, discolouration of the skin or persistent numbness seriously and to have them clarified by a doctor immediately.

Choosing an experienced specialist, careful planning as well as compliance with the aftercare recommendations contribute substantially to minimising the risks of lip augmentation and to achieving a safe, harmonious result.

FAQ: lip filler techniques

Many people have similar questions about lip augmentation and the possible techniques. They often concern the differences between the methods, the effectiveness of individual techniques or the durability of the result. In the following, the most important questions are answered concisely and clearly in order to provide orientation when choosing the appropriate method.

  • Various injection techniques are used for lip augmentation. The best known include the Russian Lips technique, the tenting technique, the linear injection technique, the fan technique, the vertical technique as well as the keyhole technique.

Each of these types pursues a different goal. While some techniques primarily build up volume, others concentrate more strongly on contour definition, elevation or a particular lip shape. The choice depends on the individual anatomy and the desired aesthetic result.

  • There is no generally „best“ technique. The most effective method is always the one that suits the individual lip shape and the desired result. For a natural volume, the linear or vertical technique is often suitable. Anyone who wants a defined heart shape may benefit from Russian Lips. Effectiveness therefore does not mean maximum fullness, but a harmonious and anatomically coherent result.

  • The term „lip filler“ basically describes the material – usually hyaluronic acid – that is used to build up volume. Russian Lips, on the other hand, is a specific injection technique. While lip fillers can generally be used for different shapes, the Russian Lips technique specifically aims at a vertical elevation and a heart-shaped contour. The difference therefore does not lie in the material, but in the placement of the filler.

  • The durability depends primarily on the material used, less on the technique itself. With hyaluronic acid, the average durability is usually between six and twelve months. Techniques with a smaller amount of material or a purely visual effect – such as the lip flip with Botox – generally last for a shorter time. Autologous fat can potentially last longer, as part of it can heal in permanently. Nevertheless, durability remains individually different.
